Apply for a new Managers Certification

To apply, you must first download and complete a Managers Certificate (new) application form [PDF, 233KB] and then upload it here.

Please ensure you have read and understood the privacy statement, signed and dated your application.

Please upload your completed Managers Certificate (new) application form.

Identification displaying your full legal name, such as your New Zealand drivers licence, passport or birth certificate

Letter from your employer detailing why you need to have a managers certificate and confirming your position and duties

Any visas you hold, such as a permanent resident, resident, student or working visa
