Notice of duty manager appointment or change

Premises information

This is the name of your premises (supermarket, bottle store, restaurant etc). It is the name on the sign above your door. It is not the name of your company.


Licensee name is the full company name or partnership name as appears at the top of your alcohol licence.

This is the role you hold at your premises e.g owner, director etc.



It is not necessary to notify the DLC or Police in respect of the appointment of an acting manager for any period not exceeding 48 consecutive hours.

Reminder: When a Duty Manager is on duty their full name must be displayed on the premises at all times.

New permanent manager (who holds a manager's certificate)
This notice should be submitted within two working days of the appointment.

The date they’ll start working as duty manager.


This is the staff member’s full legal name (as appears on their manager’s certificate, birth certificate or passport).

This is the name you call them. It could be their legal middle name or their nickname e.g Nicky instead of Nicola.


Please ensure this is filled in correctly – this allows us to properly identify the person in case of two people with the same name.

Your staff member’s manager’s certificate number (in the format: xx/CERT/xxxx/20xx).

Expiry date on their manager’s certificate.
Temporary manager (when certificate is issued, they then need to be appointed as a new permanent manager)

Note: A temporary manager must apply for a manager's certificate within two working days of the appointment.

The date they’ll start working as duty manager.


This is the staff member’s full legal name (as appears on their manager’s certificate, birth certificate or passport).

This is the name you call them. It could be their legal middle name or their nickname e.g Nicky instead of Nicola, or English name


Please ensure this is filled in correctly – this allows us to properly identify the person in case of two people with the same name.


The name of the duty manager they are replacing. Please complete a termination/cancellation notification for the person leaving.

Expiry date on their manager’s certificate.

Reason you need a temporary manager e.g. duty manager leaves, duty manager on extended sick leave etc.

Note: A temporary manager must apply for a manager's certificate within two working days of their appointment.
Acting manager (used to cover absences of permanent managers)
See section 230, Sale and Supply of Alcohol Act 2012.
This notice should be submitted within two working days of the appointment.

The date they’ll start working as duty manager.

The finish date for the date range when they're filling in as an acting duty manager.

Note: An acting manager can be appointed for a maximum of three weeks at a time and then only for a maximum of six weeks in a 12-month period.

Termination/cancellation of existing manager appointment
This notice should be submitted within two working days of the termination/cancellation.

The date they stopped working as a duty manager for you.


This is the staff member’s full legal name (as appears on their manager’s certificate, birth certificate or passport).

This is the name you call them. It could be their legal middle name or their nickname e.g Nicky instead of Nicola, or English name


Your staff member’s manager’s certificate number (in the format: xx/CERT/xxxx/20xx).

Expiry date on their manager’s certificate.
A copy of your completed form will be emailed to you. Please keep a copy as part of your Premises Record (s232) of Duty Managers which is required to be kept by all licensees. You may need to produce it to show it was sent and received.
